Delivery interruption investigation / Missed Delivery Investigation

Before investigating any delivery interruption, please first verify if the member has completed their benefit duration:

  • CountyCare Medically Tailored Food Program: Members receive varying lengths of food support based on claims-based algorithms. Approximately 80-90% of members receive food boxes, while 10-20% receive medically tailored meals. Members typically receive:

    • 3-6 months of food boxes

    • 1.5-3 months of medically tailored meals (MTMs)

If a member appears to have fulfilled their benefit, note that there is currently no option for renewal at this time.

CountyCare Emergency Meals Program: This program provides a one-time shipment of 14 pre-made frozen meals. Members are eligible for multiple rounds of this benefit if deemed food insecure. If delivery was not fulfilled, reach out to Jackie Downing to contact patient and resubmit order as necessary.

  • CalOptima: Food delivery quantities are based on the member’s risk assessment. Please refer to the assessment for the most accurate benefit details for each eligible member. If deliveries stop before the benefit is completed, reach out to the Foodcare Operations Team to contact the food provider to check on the delivery status.
