Food Delivery Status Tracking

If your member has not received their food delivery please see the below information and verify they have not completed their benefit duration before submitting a ticket.

CountyCare Medically Tailored Food Program:

Members receive varying lengths of food support based on claims-based algorithms. Approximately 80-90% of members receive food boxes, while 10-20% receive medically tailored meals. Members typically receive:

  • 3-6 months of food boxes

  • 1.5-3 months of medically tailored meals (MTMs)

If a member appears to have fulfilled their benefit, note that there is currently no option for renewal at this time.

CountyCare Emergency Meals Program:

This program provides a one-time shipment of 14 pre-made frozen meals. Members are eligible for multiple rounds of this benefit if deemed food insecure.

CalOptima:

  • Number of weeks for food benefits are based on the member’s risk assessment (high risk = 12 weeks, medium risk = 4 weeks).

  • CalOptima has a maximum lifetime benefit of 12 weeks of food - so depending on how many weeks of food members have previously received, their food referral may be adjusted to a different length to ensure they don’t receive more than 12 total weeks of food.

  • Please refer to the assessment for the most accurate benefit details for each eligible member.

  • If deliveries stop before the benefit is completed, submit a ticket for Food Support.

Santa Clara Family Health Plan (SCFHP):

To check the status of your member's food delivery have the member call SCFHP at 408-874-1929. Do not submit a ticket for an update.

Health-Related Social Needs (HRSN):

  • Vendors are assigned based on the member’s referral to determine their food service needs.

  • We send out referrals within 24 hours after a risk assessment is submitted.

  • Vendors will get in contact with the member within 7 business days to begin scheduling deliveries.
