What is the NutriQuiz?

Nutriquiz gives your patient a baseline assessment of their current eating habits and provides targeted benchmarks and goals to work towards. Nutriquiz should be used as an educational tool with patients and includes screening questions for food insecurity. NutriQuiz taken during the patients Initial Visit; retake after 2-3 months, 6 months, and 12 months to measure positive outcomes.

Why Take The NutriQuiz?

NutriQuiz is one of the main sets of data that health plans use to evaluate our performance; our contracted health plans do not have access to health metrics in FFD. Because of this, it is imperative that you support your patient in completing NutriQuiz at baseline (initial visit) and at least at 2 months so that we can continue demonstrating improvements in dietary intake. Demonstrating improvements in dietary intake and the association with improved health outcomes helps us continue to keep your calendar booked with patients.

What if the NutriQuiz link does not appear in FFD?

Please direct the member to their Foodsmart account and they can start taking their own NQ. If you believe the member's account should have a NQ link you can submit a missing NQ link ticket at the bottom of this article.

Is there a NutriQuiz for children?

No, not at this time.

Can members log into the website to see their results?

Yes! Have the member click on NQ quiz on the menu bar in the top right once they are logged into their Foodsmart.com account.

What do I do with the NQ results?

Create smart goals with the member from the NQ results.

Can you see multiple past NQ results?

No, once you re-take the NQ you only see the most recent past results.

Does the NQ results automatically populate into the FFD chart?

No, the RD can add the information to the chart but it is not required at this time.

  • Menu Bar above the Nutriquiz is the patient's Foodsmart account.

  • Last date of NutriQuiz taken appears in FFD

  • Document NutriQuiz results in the patient's chart
