Expiration and Resetting for Another Year
Elevance - Anthem Healthkeepers, Cardiometabolic Pilot, Healthy Blue/KS Expiration:
Members have a minimum of 30 days and a maximum of 2 months to use their incentives.
We cannot give replacement incentives for expired incentive codes. If they are not used within the window of opportunity, they will lose their incentive.
Incentive expiration dates follow this schedule:
Examples:
Appointment on May 1 → Expires June 30
Appointment on May 15 → Expires June 30
Appointment on May 30 → Expires June 30
All Other Program Foodsmart Bucks Expiration:
Foodsmart Bucks (Fresh Funds) expire one year from the time your Foodsmart Bucks (Fresh Fund) codes are given to you to claim it and use it in an order on Instacart!
All codes, no matter when given, expire when they reach a year after the visit was completed AND they HAVE to be used on Instacart in that same time frame.
If there is an issue with a code having an error or it expires earlier than the year, please fill out a Foodsmart Bucks ticket to have it replaced. If it is after a year, no code will be reissued.
Examples:
Visit on 12/11/24 expires on 12/10/25
Visit on 1/3/24 expires on 1/2/25
Visit on 2/12/24 expires on 2/11/25, but on 1/30/25 says it is expired or gives an error code, submit a ticket.
Elevance - Anthem Healthkeepers
Only gets two $25 incentives: $25 for initial visit and $25 for first follow up visit
No incentives reset in a new calendar year.
Expiration for these codes is a year from the visit date. They have to be redeemed and used in an order on Instacart prior to expiration.
Virtual Incentives
Virtual Incentives (virtual Walmart gift cards) expire 90 days after being issued and the expiration is explicitly mentioned under the reward in the email when sent to the member.
