PAR-Q (Physical Activity Readiness Questionnaire)

The PAR-Q feature lets you select health and safety questions to present to students before they participate in classes. It helps ensure your instructors are aware of any medical conditions or physical limitations that could affect a student's participation.


Enabling PAR-Q

PAR-Q is enabled from Account > Studio Profile. Scroll down to the PAR-Q card in the feature settings section.

PAR-Q (Medical form) Turn this on to activate the PAR-Q for adult students. Once enabled, an additional Mandatory checkbox appears — tick this if you want to require students to complete the form before they can proceed. If left unticked, students will have the option to skip it.

Child PAR-Q (Medical form) A separate toggle for child accounts. Works the same way as the adult PAR-Q — enable it and optionally mark it as mandatory. Child responses are tracked independently from the parent's.

Form appears Before / After payment Choose whether the PAR-Q is shown to the student before or after they complete their payment. This applies to both adult and child versions. The default is After.


Selecting Your Questions

Once PAR-Q is enabled, go to Manage Studio > PAR-Q to choose which questions are presented to your students.

The page is split into two tabs — Parent and Child — so you can configure adult and child questions independently.

Each available question has a Required checkbox next to it. Tick the questions you want to include. Use the Select All checkbox at the top to select or deselect all questions at once. Click Update to save your selections. Only the questions you select will be shown to students — anything left unchecked will not appear.


Available Questions

Questions are sourced from a predefined list. Studios can select which questions are relevant to their classes. Custom questions cannot be added.

Injuries & Physical Conditions

  • Are you affected by any injuries, broken bones, or torn ligaments?

  • Are you affected by arthritis?

  • Have you recently undergone surgery?

Medical & Health History

  • Do you or anyone in your family have a history of heart disease, stroke, raised cholesterol, or high blood pressure?

  • Are you affected by any medical condition (asthma, diabetes, epilepsy, etc.)?

Exercise & Physical Readiness

  • Do you participate regularly in any activity or exercise? If yes, please give details.

  • Are you or could you be pregnant?

Physical Support

  • Do you have any objections to spotting? Your instructor may use their hands to assist with moves as needed.

Other Conditions

  • Are you affected by low blood pressure or severe headaches, dizzy spells, or loss of consciousness?

Class-Specific

  • Will you be taking a bungee class? If yes, please enter your approximate weight.


How Students Experience the PAR-Q

When a student books a class for the first time, the PAR-Q form is presented either before or after payment depending on your settings. Each question is answered with Yes or No. If a student answers Yes to any question, a text field appears for them to provide more detail.

If the PAR-Q is not marked as mandatory, a Skip button is available. If it is mandatory, the student must complete the form to proceed.

The PAR-Q is shown once per student. After they have submitted (or skipped) their responses, it will not be shown again on subsequent bookings.


Viewing Student Responses

PAR-Q responses are stored against each student's profile and can be reviewed at any time.

Go to Student Management, open a student's profile, and select the PAR-Q tab. Responses are shown as question-and-answer pairs. If child accounts are linked, their responses appear under the Child sub-tab, grouped by child name and expandable for easy reading.


Child PAR-Q

When a parent books a class for a child, the child PAR-Q is presented separately from the parent's form. Each child in the booking can have their own set of responses. Child responses are tracked independently and are viewable in the student profile under the Child sub-tab of the PAR-Q section.
