Create a New Form (Blank form or template)

You can create a new form from scratch, or start from a template and tailor it.

Create a blank form

  1. Open Forms.

  2. Select New Form.

  3. In the dialog titled Create new form, enter a Form name.

  4. Select Create form.

After creation, the form opens in Form Builder.

Start from a template

When there are no forms yet, you will see a card called Explore templates with the description “Start with a template and tailor it to your needs.”

Select Explore templates to open the template picker.

In the template picker:

  • Use Search templates to find a template by name or description.

  • Filter by categories such as NDIS, HCP / SAH, Clinical, HR / Staffing, Operations, Finance, and Service Delivery.

  • Select Blank Form to start from scratch in a template-like flow.

  • Select a template to prefill steps and fields.

What happens next

The selected template creates a form with:

  • A prefilled form name and description

  • Prefilled steps and fields

  • Allow Step Navigation enabled

You can then edit everything in Form Builder.

Troubleshooting

Create form is disabled
The form name must not be empty.

Form created, but it did not open correctly
You may see a message that the form was created but the list could not refresh or the redirect could not be resolved. Return to Forms, refresh, then open the new form from the list.
