Before you start
Form Builder is not available on mobile. If you open it on a mobile-sized screen, you will be told to use a desktop browser.
The three main areas
Form Builder is split into:
Left sidebar for Fields and Steps
A canvas area where fields appear
Right sidebar for settings, publishing, preview, and share tools
Add fields
In the left sidebar:
Open the Fields tab.
Drag a field into the canvas.
You can also select Search all fields to find a field quickly and insert it.
Live form restriction for internal-only fields
When a form is live, internal-only fields are restricted. You will see a message like “Live forms can't use internal-only fields.” Some internal-only fields include staff and client selectors.
Manage steps
In the left sidebar:
Open the Steps tab.
Select Step to add a new step.
Select a step to make it active.
Rename a step from the step list.
Drag steps to reorder them.
Use the step menu to Duplicate or Delete.
A step cannot be deleted if:
It would leave you with no steps
Lock all fields is enabled
The step contains locked fields
Preview the form
Use Preview to open a preview of the form. This helps you confirm step flow and required fields before publishing.
Saving
Form Builder saves your changes as you work. Some actions require the form to be saved first, such as publishing live.
Using Forms in Client Documents
We’ve expanded our form capabilities to give you more flexibility when documenting client care. Previously limited to Care Plans, you can now create, submit, and edit form-based documents in any supported document folder within a client’s record.
Other Features
Locking Fields: You can now prefill and lock fields so users can't change them.
Embedding: Forms can now be embedded into other websites.
Internal Publishing: You can now publish forms for "Internal Use Only."
Troubleshooting
I cannot add or insert fields
You may not have an active step, or Lock all fields is enabled.
I cannot delete a field
Fields cannot be deleted if the form is locked, or if the field itself is locked.
