Creating Incident Forms

Instead of manually typing up incident after the fact, you can now link Imploy Forms directly to your incident workflows. It allows you to collect data automatically routing submissions into your incident log.

Creating an Incident Form via the Template Center

The fastest way to build an incident intake process is through our Template Center, which comes pre-configured with mapped system logic.

  1. On the left-hand main navigation sidebar, scroll down to the Business section and click on Forms.

  2. Look at the top right corner of your screen and click the black + Form button. This action overlays the Template Center window.

  3. Look under the Record Creation sub-header on the left side of the modal, and click on Incident Report Form.

  4. Give your form a name in the Form name field (e.g., Incident Report Form).

  5. The system will automatically map core system attributes (such as Title, Description, Date reported, Priority, Category, Reported by, Assigned to, and Location) as well as your custom incident attributes.

  6. Choose whether the form is internal-only or public by toggling the Internal / Public switch in the top right corner.

  7. Click the black Create incident form button to finalise your form.

Sharing Publicly vs. Internally: Depending on how you configure your form's publishing settings, you can open the form from the Incidents sidebar, copy its unique link, and use it as a simple incident intake process for staff, teams, or external reporting where needed.

Converting an Existing Form into an Incident Form

If you already have a form designed that you want to make it incident form:

  1. Go to the left navigation sidebar under Business and click Forms.

  2. Find your existing form from the list and click it to open the Builder view.

  3. Locate the Use as incident form toggle in the form configuration panel.

  4. Manually map your custom form questions to our core system variables (such as Title, Description, Date Reported, etc.).

  5. Click Save.

Once saved, these specialised intake forms live directly inside your Incidents sidebar under the Incident Forms section.
