RBWH PGY1 Education Activity Log Form

Shows how to open complete and access the PGY1 education activity log form to submit education activities over the year

At RBWH you will need to submit your education activities to the Medical Education team using the activity log form in Med App. You don't need to submit a form for events you have attended and used the attendance QR code - this form is for everything else.

Here is a step by step guide (along with a video run through) on how to access and use this form. Other help centre articles will have more general information on Med App and the forms feature.

Where to find the form template?

You will find it on the main 'guide' page of the app under the 'Forms' tile.

Look for the PGY1 Education Activity Log form.

Start a new form

Once you've tapped on the template, tap the 'start new form' button.

Tapping the 'view forms' button will take you to all completed and draft versions of that form.

Note, you will also see the number of forms you have completed or are in draft for that template.

Complete all sections

Go through the form and complete all required sections. This includes the topic of the activity and at least 2 sentences on the learning points for that activity.

Save as draft

If you want to come back and complete a form later, just tap the 'save as draft' button.

Complete on a desktop browser

If you do want to use a web browser on a desktop you can tap the desktop icon in the top right when viewing a form to convert to a web browser.

You will then be given a code that you can enter into the URL: https://web.med.app/form

TIP: If you have already entered information in make sure you have saved as a draft to ensure the info is reflected in the transferred version.

Where to find draft and completed forms?

You can either go to the 'Forms' tile, then tap the template and then 'view forms'. Or you can go to the 'more' tab and then 'My Forms'. You will then have tabs for 'for action' forms and 'all forms'.
