This is a form template that is available under the 'Forms' tile on the main guide page of the app (slightly different to the My Forms section in the 'more' tab).
This is a template that allows you to submit multiple versions of. In this case you can submit a form per activity. The app will show you how many you have submitted and how many are in draft. You can also jump in to see all the previous ones you have submitted.
How To Use
Find the 'Forms' tile on the main guide page of the app (the one with all the tiles).
You should see the CPD activity form template. On it you will see how many you have submitted and how many are in draft.
Tap on the template.
It will show you options to
Start a new form
This will start a new version of the form. Remember you can submit multiple versions of this one. So you can submit up to 50 of them per template.
View forms
This will take you to the 'My Forms' section for the template and will show you a list of all the ones you have submitted for that template as well as the ones currently in draft.
If you want to start a new one, go back to the tile to tap on the template and the 'start new form' button
Once you're in the form, answer the questions and enter required information.
Once you're done, you can submit it and it will record a completed form.
