This form will be assigned to you specifically - so it will show up under the 'more' tab in My Forms.
Complete the self assessment section first.
Tip: As this is a reasonably long self assessment you can transfer to the desktop if you want.
Tap the 'desktop' icon to transfer to a browser. This will give you a code which you enter in via the provided URL and it will bring up the form
Make sure to save as a draft before transferring to a browser, or after completing information in the browser.
Tap continue then assign to your preceptor or correct supervisor/assessor
You can assign to them by phone or email, just select the option and enter in the email address or the phone number.
Once your preceptor/NUM/supervisor completes their section of the form you will get a message to complete your final section.
You may have a discussion with them to run through the assessment responses or comments.
Once that is finalised it will be assigned back to you for review, signature and submission.
You can then review the preceptor/NUM/supervisor's section before signing and submitting the form.
