This form will be assigned to you specifically - so it will show up under the 'more' tab in My Forms.
Select which month the discussion relates to (1, 2, 6)
This is a 'single section' form so don't submit until you are finished with all the entries. Just 'save draft' each time you add to it.
Tap 'add entry' to enter a new record
Complete the necessary sections.
For the NUM/MUM/NE?CNE/CME/Preceptor signature/initials - just show them your phone and get them to sign their initials on the screen directly.
This will form a record for you so be honest with the responses as it can be checked with the responsible supervisor if required.
Save as a draft after you entered in the entries
You can continue to enter multiple entries over time.
DO NOT submit until you have entered everything in.
Once you are done you can submit and get a PDF record of those activities.

Show them the phone and get them to pop their signature into that field (it won't have their name on the bottom of it, it should be entered as an earlier field above).
Don't submit right away
Save as a draft to come back and add additional entries as is needed - you can then submit and get a full PDF report of the entries in one hit.
