St George GradStart: Completing the Learning & Development Goals form (CNEs and NUMs)

How CNEs, Preceptors and NUMs review and sign a GradStart RN's Learning & Development Goals form from an email link. No Med App account needed.

Who this is for

This guide is for Clinical Nurse Educators (CNEs), Preceptors and Nurse Unit Managers (NUMs) at St George Hospital who support registered nurses in the GradStart Program.

GradStart RNs complete their Learning & Development Goals in Med App and send the form to you by email to review and sign your section. You do not need a Med App account or the app. Everything works from the link in the email.

Why the form matters

Each GradStart RN identifies their own learning goals in collaboration with their CNE. This must happen within six weeks of the start of each clinical rotation. The goals:

  • are aligned with the GradStart Performance Development Review

  • are used as evidence of ongoing professional development

  • are reviewed at the 3-month appraisal, where new goals can be set

When the form is complete, a copy is sent automatically to the Transition Coordinators.

How the form moves between people

  1. GradStart RN enters their name, ward or unit, and at least five learning goals. Each goal covers the SMART goal (learning need), the strategy and resources, the expected date of achievement, how achievement will be measured, and progress at review.

  2. CNE or Preceptor receives an email, reviews the goals, then adds their name and signature.

  3. GradStart RN receives the form back to confirm whether they agree or disagree with the strategies and review dates, add any comments, then date and sign.

  4. NUM receives an email for final review, then adds their name and signature.


Completing your section

1. Open the email

You'll receive an email from Med App letting you know a form is waiting for you. Select the link to start your review.

Can't find the email? Check your Junk or Clutter folder. Ask the RN to confirm they entered your correct NSW Health email address. If it still hasn't arrived, the RN can send it again.

2. Verify your details

Enter your details when prompted. This links your signature to you.

3. Review the form

Click through each step of the form. CNEs and Preceptors should check that each goal:

  • is specific and measurable

  • has a realistic strategy, with named resources such as people, learning packages or in-services

  • has an achievable date

  • has a clear way of measuring achievement

Ideally you'll have discussed the goals with the RN before the form reaches you.

4. Sign and submit

Enter your full name, sign in the signature box and submit. The form moves to the next person automatically.

Drafts - You can save a draft and come back to it later.

Another device - You can open the link on another device, such as your phone, to finish.

What if the goals need changes?

If critical changes are needed after the form has been submitted, ask the Med App dashboard managers to retract the form so the RN can update and resubmit it.
