Emotional Support Animal (ESA) Letters

MindDemand offers Emotional Support Animal (ESA) letters for patients with qualifying mental health conditions. ESA letters are issued by our providers when they determine that an Emotional Support Animal may provide therapeutic benefit as part of your treatment plan.


Who Can Request an ESA Letter?

Both new and existing patients can request an ESA letter through MindDemand.

  • Existing patients: If you already have an established diagnosis with a MindDemand provider, your provider can evaluate whether an ESA letter is appropriate and issue one during a scheduled visit.

  • New patients: If you are new to MindDemand, you will need to schedule an appointment with one of our providers for a clinical evaluation. During this visit, your provider will assess your mental health history, current symptoms, and whether an ESA letter is clinically appropriate.

Please note: ESA letters cannot be issued without a clinical evaluation. The decision to issue a letter is based on your provider's professional judgment.


What You Will Need to Provide

When an ESA letter is approved, you may be asked to provide some basic information about your animal so the letter can be prepared accurately:

  • Type of animal

  • Animal's name

  • Gender of the animal

  • Color or identifying description


Fees

ESA letters are considered an additional service and involve a separate fee:

  • $99 for the first animal

  • $49 for each additional animal

Payment must be completed before the letter is issued. This fee is separate from any appointment copay or self-pay charges.


Validity and Renewal

ESA letters issued through MindDemand are valid for one year from the date of issue.

If you need updated ESA documentation after your letter expires, you will need to schedule a follow-up appointment so your provider can confirm the ESA remains appropriate as part of your ongoing treatment plan.

To request an ESA letter:

  1. Existing patients: Contact us to schedule an appointment with your provider, or bring it up during your next visit.

  2. New patients: Book your appointment to get started with a clinical evaluation.
