How to Manage Patients Without Cell Phones in Phamily

Patients don't need to text to enroll in CCM! Learn more about enrolling patients without mobile numbers and managing their care in Phamily.

Sometimes a patient is a great fit for CCM but doesn't have a mobile number or would prefer not to text. You can still enroll this patient in CCM and log their care management time right in Phamily!

How to add a patient without a mobile number

  1. Log in to Phamily and click the Add Patient icon in the upper right corner of the page.

  2. Add the Patient's Name and DOB > Click the Add Home Phone and MRN button and Enter the Home Phone Number

  3. Click Add Patient

How to enroll a patient without a mobile number

  1. Navigate to the patient's profile in Phamily. Click ENROLL.

  2. The pop-up window that appears will be pre-populated with a standard CCM welcome message. Click the dropdown next to ENROLL & SEND MESSAGE, then select ENROLL WITHOUT A WELCOME MESSAGE. This will mark the patient as enrolled without sending a welcome message.

How to update the patient's Phamily monitoring level

  1. On the patient's profile, click the circle to the left of the patient's name. Then click MANUAL. This excludes the patient from ongoing check-in text messages.

Tips for ongoing care management

Filter patients by check-in status by clicking the circle in the upper left corner of the patient list, then choosing MANUAL. This allows you to view all patients who are excluded from automatic check-ins.

We recommend the following for ongoing management of Manual patients:

  • Plan to call these patients at least twice a month to proactively check in. It's helpful to explain to patients that you'll be calling them throughout the month during enrollment. You can have them provide days/times that work well for a phone call and even take note of the practice number so they know to take your call.

  • Use Phamily's tagging feature to stay organized: you can mark patients in need of a phone call or even organize patients by week. We've seen some practices split their Manual patients into groups by week and call throughout the month.

  • Use Phamily's Additional Remarks field to make note of patient preferences. Great if you have a patient who prefers to be called on Tuesdays or if you have a patient with a caregiver.

  • Use Phamily's Tasks feature to set a reminder to follow up. This is a great resource if you call a patient and they don't answer or ask you to call back.

  • Make sure to log time for all time preparing for the call, speaking on the phone and performing activities for the patient!
