Overview
RepeatMD in-app purchases can include a per-transaction fee shared between the patient and your practice. How the fee is split depends on your practice’s configuration. This article walks through charge scenarios so you can explain totals to patients and reconcile payouts in your shop Summary Report.
This article is for practice owners, managers, and billing contacts. It covers fee math on patient purchases processed through your RepeatMD marketplace (not RepeatMD platform subscription billing).
Before You Start
Transaction fees apply to in-app patient purchases (treatments, packages, memberships, products) when fee-pass-through is enabled for your practice.
Fee splits are configured during onboarding or account setup. Contact your RepeatMD account team if you are unsure how fees are split for your app.
Practice fees appear in the shop Summary Report in your Admin Panel.
Sales tax, when applicable, is added on top of the patient’s total at checkout.

Why It Matters
Transparent fee math prevents checkout surprises and payout confusion. When your team understands how list price, patient fees, and practice fees interact, you can:
Answer patient questions about why their total is higher than the listed price.
Reconcile Stripe disbursements against the shop Summary Report.
Compare card vs. Affirm economics before recommending financing at checkout.
Escalate refund questions to the correct article and Support workflow.
How It Works
Split fees. When enabled, the per-transaction fee can be split equally between the patient and the practice. Each party pays their portion of the configured percentage.
What the patient pays: The patient pays the list price plus their share of the transaction fee, plus sales tax when applicable.
What the practice receives: The practice receives the list price minus the practice’s share of the transaction fee. The practice fee is shown in the shop Summary Report.
Card vs. Affirm: Card and Affirm transactions use different processor rate structures. Affirm practice fees are calculated on the total amount financed, which can differ from card fee math.
$0.30 note (card): RepeatMD does not collect the $0.30 fixed component from patients on card transactions.
Refunds. Refunding an order may reverse or adjust fees depending on whether the refund is full or partial and whether the purchase was card- or Affirm-financed. Use Issue full and partial refunds (Admin Panel) for procedures.

Card purchase example (fee split 50/50)
Scenario: Tim pays by card for a facial. List price is $79. The practice splits the per-transaction fee evenly so each party pays 2.5%.

Log in to the Admin Panel.
Open your shop Summary Report (or equivalent commerce reporting for your account).
Locate the practice fee column for the transaction you are reconciling.
Compare disbursed amounts to your Stripe payout records.

Q: Why is the patient’s total higher than the listed price?
A: When fee pass-through is enabled, the patient pays their share of the per-transaction fee on top of the list price, plus tax when applicable.
Q: Where do I see the practice fee?
A: In the shop Summary Report in your Admin Panel.
Q: Are card and Affirm fees calculated the same way?
A: No. Affirm practice fees use financing-based rates (for example, 3.99% + $0.30 of the amount financed). Card fees follow card processor rates on list price.
Q: Is the $0.30 fixed fee charged to patients on cards?
A: RepeatMD does not collecting the $0.30 from patients on card transactions. The $0.30 is tied to credit card and/or Affirm processing fees.
Q: How do I issue a refund?
