As part of onboarding, we import the facility’s current/active therapy caseload, including the most recent Evaluation or Recertification available for each patient. This provides a clean starting point in the system, based on the documentation shared at setup.
Because we import one current document per patient, a few important workflows are automatically triggered.
How Transitional Evaluations Are Created
When an Evaluation or Recertification (PDF) is imported, RESTORE-Insights uses that information to generate a Transitional Evaluation in the system. If no plan of care was imported for a patient, therapists can also manually create a transitional document for that resident within their Episode of Care.
As part of the onboarding process, your team should validate the information in the transitional documents:
Navigate to the patient's Episode of Care (EOC). From there, you can use the tabs to review documentation for each discipline. Within the EOC, find your document and click View Document to review before making changes. To edit, first click the Re-assign Therapist option, select the desired therapist, and then they'll have Edit access.
If any information needs to be adjusted to reflect the most current Plan of Care, therapists can update it directly within the Transitional Evaluation before signing.
Ensure precautions and contraindications are updated as appropriate.

Note: Only Evaluators can edit and sign Transitional Evaluations.
Why Progress Note & Recertification Alerts Appear
During the import, certification dates from your provided documentation are entered into the system. RESTORE then uses those dates to automatically calculate:
Progress Note (PN) due dates
Recertification due dates
As a result, you may see alerts shortly after go-live. These alerts are not errors — they reflect timelines based on the certification dates included in the imported data.
Please note:
Progress Notes and visit counts are not imported.
Progress Notes should be completed as visits occur moving forward.
Alerts are designed to help you stay proactive and compliant.
What This Means for Your Team
The import creates a structured starting point, but your team remains in control of validating and updating clinical details to ensure everything reflects each patient's current Plan of Care.
Taking a few moments to review Transitional Evaluations and monitor alerts will ensure accurate documentation, compliance, and a smooth transition into RESTORE-Insights.
