You can add multiple custom choice boxes to your online form! This is great for multiple choice questions or adding specific options you want your patients to be able to choose from.
Let’s take a look at the Choice Box Settings:
1. First click on the Choice Box option in the Add section
2. Check Box Settings Screen
How to name your Choice Box:
Field: To create a new Field, select -new field- in the dropdown.
Name the new field by entering the name in the Label bar. Label is what is displayed on the form.
The Name bar will auto-populate. Name is an internal value used to identify the field for use on things like replacement strings.
3. Select Input Type:
The Choice Box option gives your patient the option to select multiple boxes (e.g. “check all that apply”)
The Radio option makes it so that your patients can only select one option (e.g. “yes or no”)
4. Required: Make the Choice Box field mandatory.
5. Choice Box placement:
Label Position: Here you can select for your label to appear on the Top of the Choice Box or to the Left of the choices.
Alignment: Here you can adjust the alignment of the Choice Box content within the designated space, (e.g. left, right or center)
Field Width: Here you can adjust the length of your Choice Box bar, (e.g. with the Full option, the Choice Box will run the full length of the form, and with the Half option, the Choice Box will run to half of the page, leaving room for another form field.)
6. How to add Choice Box options:
Label: Here type the name of the selection you want to appear on the form.
Value: The Value field will auto-populate with what you typed in the Label field; this is an internal value used to identify the field.
To add more choice box selections, click the +Add Option button.
7. When you’ve finished configuring the Choice Box, click the OK button.
8. The Choice Box will appear in the Step Designer. Drag the Choice Box to where you want it using the drag-able icon on the top left of the bar.
