📝 How to Add a Text Box or Choice Box to a Form

Collect more meaningful and structured responses from patients.

Adding a Text Field to a Form

Text Fields are perfect for collecting names, emails, phone numbers, or even detailed responses like medical history. Whether you're keeping it short or asking for a paragraph, you’ve got flexible options!


🛠️ Text Field Settings:

  1. Naming Your Field

    • Field: Select –new field– to create one

    • Label: What your patient sees

    • Name: Internal name for things like automations or replacement strings

  2. Input Type Options

    • Plain Text: Great for names or addresses

    • Email: Ensures proper email formatting

    • Password: Masks what’s typed

    • Phone: Requires a valid phone number

    • Text Area: Larger space for detailed responses (ex: "Tell us about your symptoms")

  3. Required Toggle
    Make the question mandatory by turning on the Required toggle

  4. Field Placement

    • Label Position: Top or Left of the text box

    • Alignment: Left, Right, or Center

    • Field Width: Full or Half (lets you pair this with another field)

  5. Click OK to apply your settings

  6. Drag your Text Field into position using the handle on the top-left corner of the bar


✅ Add a Choice Box to a Form

Looking to collect multiple choice or single-option answers? The Choice Box lets you offer custom options to help streamline responses from your patients!


🛠️ Choice Box Settings:

  1. Click the Choice Box button in the Step Designer

  2. Field Naming

    • Field: Select –new field–

    • Label: What your patient sees

    • Name: Auto-generated internal name

  3. Select Input Type

    • Checkbox: Allows multiple selections ("Check all that apply")

    • Radio: Allows only one selection ("Yes" or "No")

  4. Required Toggle
    Make it required if patients must answer

  5. Field Placement

    • Label Position: Top or Left of the Choice Box

    • Alignment: Left, Right, or Center

    • Field Width: Full or Half layout option

  6. Add Options

    • Label: Type what you want the patient to see (e.g. “I have back pain”)

    • Value: Auto-filled; used internally

    • Click + Add Option for more selections

  7. Click OK to save the Choice Box

  8. Drag and drop it into the desired spot using the top-left icon


💾 Final Step: Save Your Form

After you're done customizing your fields, always click Save Form at the bottom left of the Step Designer to lock in your updates.
