📄 How to Create a NEW Form in Aloha

Build custom digital forms for patient intake, feedback, and more

✨ When You Need This Guide

Use this when you want to:

  • Create a new form from scratch

  • Customize digital intake or feedback forms

  • Make a questions required

  • Get notified when patients complete a form

  • Start a follow-up campaign automatically

Common questions this answers:

  • “How do I build a new form in Aloha?”

  • “Where do I edit what patients see on the form?”

  • “How do I get notified when someone fills one out?”

  • “Do I need to update my website every time I make a change?”


🚀 Create a New Form

  1. Go to the FORMS tab > SHOW FORMS page

  2. Click the + Add button (top right)

  3. Fill out your form settings

    • Form Name: Choose something clear and patient-friendly (e.g. New Patient Intake Form)

    • Layout Options:

      • Interview: Patients answer one question at a time

      • Page: Patients see grouped sections at once


⚙️ Submission Settings

You can automate what happens after a form is completed:

  • Tag: Add a tag to every submission (great for bulk messages or tag applied campaigns)

  • Confirmation Redirect: Send patients to a specific webpage when they finish

  • Send Notification: Email yourself when a form is submitted

  • Start a Campaign: Automatically trigger a campaign based on the submission

Click Add Form when you're done—this unlocks Embed Settings and Step Designer.


🌐 Embed Settings

  1. Privacy Policy Options
    Choose what to show patients before submitting:

    • Do not show

    • Redirect: Paste the URL of your policy

    • Custom Text: Add your full policy text directly

🔐 If you select Redirect or Custom Text, patients must agree before continuing.

  1. Copy Your Embed Code

    • Click Copy Embed Code

    • Or email it directly
      You only need to embed the form once—any edits later will update automatically!


🧩 Use the Step Designer

Now it’s time to design your form with the tools at the bottom of the Step Designer:

✨ Once your form is designed, you can embed it on your site or send it directly via email or text—giving patients an easy, stress-free way to complete paperwork before they arrive.


✅ Make a Form Question Required

Want to make sure a patient must answer a question before submitting?

  1. Go to Forms > Show Forms

  2. Click on the form

  3. Scroll to the question > click the gear icon

  4. Toggle "Required" so it shows a blue check

  5. Click OK, then Save Form

🔁 No new embed code is needed.
📎 If the change doesn’t show right away, try clearing your browser cache or opening in incognito.
