🚀 Create a New Form
Go to the FORMS tab > SHOW FORMS page
Click the + Add button (top right)
Fill out your form settings
Form Name: Choose something clear and patient-friendly (e.g. New Patient Intake Form)
Layout Options:
Interview: Patients answer one question at a time
Page: Patients see grouped sections at once
⚙️ Submission Settings
You can automate what happens after a form is completed:
Tag: Add a tag to every submission (great for bulk messages or tag applied campaigns)
Confirmation Redirect: Send patients to a specific webpage when they finish
Send Notification: Email yourself when a form is submitted
Start a Campaign: Automatically trigger a campaign based on the submission
Click Add Form when you're done—this unlocks Embed Settings and Step Designer.

Privacy Policy Options
Choose what to show patients before submitting:Do not show
Redirect: Paste the URL of your policy
Custom Text: Add your full policy text directly
🔐 If you select Redirect or Custom Text, patients must agree before continuing.
Copy Your Embed Code
✨ Once your form is complete, use the embed code to add it to your site or share it with your web developer.
Note: If you want the form to be accessible only via a direct link, we recommend embedding it on a hidden page.
🧩 Use the Step Designer
Now it’s time to design your form with the tools at the bottom of the Step Designer:
✅ Make a Form Question Required
Want to make sure a patient must answer a question before submitting?
Scroll to any question > click the gear icon
Toggle "Required" so it shows a blue check
Click OK, then Save Form
No need to re-embed! Your changes will sync automatically to your live form.
Note: If the updates aren't appearing, please refresh your cache or try an incognito browser.
