Why is it important?

The bow-tie visualization is an ideal method of displaying all elements within a hazardous scenario.  Among other uses, each bow-tie can demonstrate the impacts of bypassing safeguards, the significance of threats and how threats and consequences are associated. Use the Hazardous Scenario Viewer to plug into MOC (Management of Change) processes or make personnel more aware of entire scenarios in one easy to understand and interactive visualization.

How does it work?

Each cause/consequence pair from the PHA (Process Hazards Analysis) is assigned a Bow-tie Center value which summarizes the unwanted event.  All cause/consequence pairs which share a common Bow-tie Center are grouped together in the visualization. Protection layers are also added to visualization linked to their associated cause/consequence.

Overview

The Hazardous Scenario Viewer gives operations a fully searchable catalog of bow-ties specific to their site. These aren't generic scenarios, these are equipment specific scenarios which teams can use to troubleshoot and use for pre-work planning.
