The prefilled form is generated when you complete the 1583 questions in the customer portal on the "To-Do" tab. If you use the pre-filled form, you will only need to add info to the highlighted boxes in yellow. There are also additional documents, including a terms of service, that are attached and need to be reviewed and signed.
First, complete the questionnaire:
After the final question, you will find instructions on completing the 1583 as well as a tutorial video. After reading the instructions and watching the tutorial video, select "Notarize Now."
You can select an online notary or in-person notary. If you select the online notary, you will be emailed a link to join your online notary session. If you select an in-person notary, you will see additional instructions like this. If you notarize in person, when you have your completed documents, you will upload them by selecting "Upload Documents." However, to download or print your pre-filled 1583, simply press "Okay."
Last, go to the Documents tab and download your pre-filled 1583 form by clicking the green download button here:
