How to document evaluations for Medicaid billing

If your district requires documentation for Medicaid billing, SLP Toolkit makes it easy to track evaluations using our built-in tools. Here's how to ensure your evaluation data is properly documented and accessible for reporting needs:

πŸ”Ή Step 1: Create a new one time session

  1. Navigate to the Daily Data tab.

  2. Right click your mouse and then click 'New Therapy Session'

  3. Select the student you evaluated from the drop down list.

  4. Click on the start/end time to open the session.

  5. Add any details for the evaluation in the Student Notes section.

πŸ”Ή Step 2: Fill in Evaluation Report Details

  1. Navigate to the Reports tab.

  2. Select the Data - Billing Export report from the left-hand side.

  3. Select the student you evaluated from the Students section.

  4. Select the time range for when the evaluation took place and click Generate Report.

  5. Find the session that corresponds with the evaluation you completed. Use the drop-down menus to select the Service Type, Place of Service, and Observation. If your specific option isn't listed, feel free to type your entry directly into the open field. See drop-down options below:

  • Service Type

    • 92521 β€” Evaluation of speech fluency (e.g., stuttering, cluttering)

    • 92522 - Evaluation of speech sound production (eg, articulation, phonological process, apraxia, dysarthria);

    • 92523 - Evaluation of speech sound production (eg, articulation, phonological process, apraxia, dysarthria); with evaluation of language comprehension and expression (eg, receptive and expressive language)

    • 92524 β€” Behavioral and qualitative analysis of voice and resonance

    • 92605 β€” Evaluation for prescription of non-speech-generating augmentative and alternative communication device

  • Place of Service

    • School

    • Telehealth

  • Observation

    • Cooperative

    • Uncooperative

    • Disruptive

    • Responsive

    • Unresponsive

🧾 What Happens Next?

  • The evaluation will now appear as a logged billable service, and can be exported in a .csv spreadsheet file.

  • If your district shares SLP Toolkit Billing reports with your Medicaid biller, then your evaluations will automatically be pulled into the report.
