How do I create a consultation form?

To create a consultation form:

To create your own:

Select + Custom section
Add a title to your section
Click + Question to add a question to the section
Add the text for the question you would like to ask
Turn on the Required toggle if you choose to make the question mandatory
Select a Question type
Choose from text, long text, single select, multiple select or images for that question
Press save
Press select treatments
Toggle on the treatment(s) you wish to apply the form section to, or select all treatments. This means the form will automatically be selected for a booking that includes these select treatments.
Before saving, if you wish to preview the form, select Preview on web
Press Save

To select from our templates:

(note, our templates can be edited)

Press Select for the template you would like to use
If you wish to edit a particular question, click on the question and make your changes
To re-order the questions in the template, hold the dots to the right of the question and drag and drop to re-order
To delete a particular question, swipe right and press to delete
Once you are happy with the questions, press select treatments
Toggle on the treatment(s) you wish to apply the form section to, or select all treatments. This means the form will automatically be selected for a booking that includes these select treatments.
