Ensure your consultation form is attached to a treatment:
Go to Menu >> Booking System
Click on Consultation forms
Select + Add section
Choose from our templates or create your own
Once you have created your question press Select treatments
Toggle on the treatment(s) you wish to apply the form section to, or select all treatments. This means the form will automatically be selected for a booking that includes these select treatments.
Before saving, if you wish to preview the form, select Preview on web
Press Save
How to change the consultation form on a booking-by-booking basis for a client:
When you create or accept a booking containing the treatment(s) selected above, the relevant form section(s) will be applied to the booking
To change the form section(s) that are sent, click edit forms
Toggle on or off the sections you wish to send to this particular client
Press Save
Upon creation or acceptance of the booking, an email will be sent to your client automatically to confirm the booking, with a link to the consultation form
An SMS will also be sent if Consultation Form Reminder SMS update is enabled
