Parental consent forms or forms to be completed on behalf of children

Please read, before using custom checklists:

Custom checklists are a great way to share forms with parents and/or educators/childminders, to complete on behalf of children (e.g. safeguarding forms, consent forms etc).

However, if you'd like to create other forms such as developmental milestones, we recommend using learning journals or the developmental milestones section. Click here to learn how to check off milestones quickly.

Tutorial on Creating Custom Checklists

Please Read, Before Watching the Video:

  1. The schedule option for custom checklists is no longer available.

  2. You can always make the checklist private after the relevant parent(s) has completed it, by selecting 'do not share.' We recommend downloading the document with signatures before making it private.

  3. Before saving a checklist, cross check that you are sharing with the correct recipients (i.e. parents or educators), as this cannot be edited afterwards.

  4. If you "un-share" a checklist all signatures are deleted. We recommend downloading the provided PDF report that shows all the signatures, before un-sharing.
