Creating New Forms (7.1/7.2): A Simple Upload Form

I need to create a place where my employees or contractors can upload an image of a license, certification, or some other document. How do I create it?

An admin can create a new upload form in WorkBright. Upload forms are ideal for asking the employee to upload an image of a license, certification, or photo. Here’s the step-by-step:

Go to Onboarding at the Top > Forms on the Left > + New Form

On the next screen, you can name the form, provide additional instructions, and set the deadline. When you reach “Form Body, select, “None,” and under “Employee Submission Type,” select “Upload Document(s).”

By selecting “None” and “Upload Document(s),” you will present an upload button to the employee that allows them to upload a document.

OPTIONAL: Documents with more than one page or side

If you want the employee to upload more than one image within the same form, you can! In the +New Form Page next to “Employee Submission Type,” an option will appear to “Upload Multiple Documents” if you select “Upload Document(s)”:

Select Multiple Files, and a pop-up window appear to name each file that you are requesting from the employee:

NOTE: If you ask for the employee or contractor to upload more than one file in a given form, the employee will be REQUIRED to submit a file for each request. Only use this option if every employee must submit all files.

Adding Questions Alongside the Upload

To add one or more questions alongside the upload, follow this additional guide. Adding fields to your upload form -- such expiration date and issuing authority -- can be beneficial in that your staff will then input reportable data alongside their image.
