Creating New Forms (1.1): A Signature-Form on a PDF in Which the Onboardee Signs and Dates a Specific Line on the Form

I have a PDF document that I need my onboardee to: Read, Sign, and Date. The signature and date need to be placed on a specific line on the form.

This is a form that requires prep in Adobe Acrobat Pro to function properly in WorkBright. If you do not have Adobe Acrobat Pro or do not wish to edit the PDF prior to uploading it into your WorkBright, or if your form requires more complex mapping, simply submit your form to our forms specialists here!

Placing the signature and date on a specific line on the form requires preparing the form in Acrobat Pro prior to loading into WorkBright. In Acrobat Pro, choose “Prepare Form” from the tools menu and add the following fields to the form:

  • A digital signature field

  • (If applicable) A text field for the date line

  • (If applicable) A text field for the signer’s printed name

Once you've added the fields, you'll need to code the Name and Date fields. Change the name of the Name field to “a4s_name”; change the name of the Date field to “a4s_date.” These are codes that will tell WorkBright to map the signer’s name and the date of signing to specific places on the PDF submission.

NOTE: To control the formatting of the name and date (such as font, font size, and font color), open the field in Acrobat and select “Appearance” under Text Field Properties.

Once you’ve edited the document appropriately, save the PDF.

Go to Forms > Forms > + New Form

On the next screen, you can name the form, provide additional instructions, and set the deadline. When you reach “Form Body,” upload your edited PDF, then select “Digital Signature” under “Employee Submits.” Make optional as necessary and click “Submit” at the bottom.
