How to Log an Incident

You can now log workplace incidents directly in Skillko, complete with descriptions, documents, and staff involvement. Our new flag system helps you stay on top of repeated issues with automatic Gray, Yellow, and Red alerts.

Accessing the Incidents Log

  1. From the main taskbar, click Training & Compliance.

  2. Scroll to the bottom of the page and select Incidents.

Creating a New Incident

Click the New Incident button at the top of the Incidents page.

Complete the incident details:

  • Incident title

  • Brief description

  • Date

    • This will default to today’s date, but you can change it using the date picker.

  • Staff member involved

Add a Report:

  • Upload supporting documentation (e.g. PDFs or related files)

  • Add any additional written information relevant to the incident

Once all required information is completed, click Save.


Viewing the Incident on a User Profile

After saving the incident:

Click the eye icon next to the person involved.

You’ll see a grey flag on their profile, indicating at least one incident has been logged.

Scroll down the profile to the Incidents section to view a log of all incidents associated with that user.

From here, you can:

  • View incident details

  • Edit incidents

  • Delete incidents if required


Logging an Incident Directly from a Profile

You can also log incidents directly from a user’s profile without going through the Incidents log.

  1. Go to Profiles from the main menu.

  2. Search for and open the relevant user.

  3. In the Incidents section, click the green “Log Incident” button.
