There may be instances where completing an assessment on Carelever Forms is not possible. This could be due to various factors, such as internet connectivity issues, lack of necessary resources, or the absence of a specific Carelever Form for the required assessment. To ensure your clinic can still proceed, you can access the paperwork copy via your Carelever Portal by following the steps below.
Locate the Referral: Log in to the Carelever Portal and find the referral you wish to process.
Verify ID:
ID verification is mandatory before downloading paperwork.
Until the ID is verified, you can view the referral components by clicking Preview.
Access Fill and PDF Features:
Once ID verification is complete, the referral will update, and the Fill and PDF buttons will become available.
If Forms Cannot Be Completed Online:
Click the PDF button for the service to download paperwork.
A pop-up will appear asking, "If you still want to download the PDF paperwork, print and fill out with pen and paper, let us know why?" Enter your reason, click Submit, and the PDF will open in a new tab for download and printing.
Don’t Forget the Consent Form:
Repeat the above steps to download and print the Consent PDF alongside the service paperwork.
Complete and Submit Results:
Complete the medical assessment and send the results manually to our results team. Instructions for this are available at Sending Results Manually | Affiliates Help Center
