Booking Confirmation and Carelever Forms Access
After making a booking at your clinic, Carelever will email the details of the assessment and a link to the relevant Carelever Forms to your nominated address.
Loggin On
KINNECT will provide your clinic’s username and password during the onboarding process. If you need assistance logging in, please contact our KINNECT Affiliate Relationship Team at 02 9099 1053 or email affiliaterelationship@kinnect.com.au. Once logged in, you can view upcoming bookings for today, tomorrow, this week, or next week.
Accessing Carelever Forms
To access Carelever Forms, click on the referral and complete the ID Verification process. Follow this helpful article for detailed instructions.
Each referral will detail the specific components of an assessment booked with your clinic.
If you are unable to complete results using Carelever Forms, once you have completed the ID Verification Process you can select the “PDF” option to access the printable version of paperwork.
Submitting Results
Click the “Fill” link on a referral to open the electronic Carelever Form in a new tab. The applicant’s basic details will pre-populate in this form.
Incomplete forms cannot be submitted, ensuring all important components are addressed during the assessment.
Once all components are correctly filled out, click the “Submit” button to send the results to our Results Team.
Updating Appointment Status
After submitting the forms and completing all components of an assessment, return to the referral page. Update the appointment status from 'Confirmed' to 'Attended' by clicking on Update Status and confirming the applicant has attended their appointment.
