Tracking Referrals Made Easy with Careflow

At Careflow, we care about making the process of tracking referrals as easy as possible. That's why we've spent time studying the nursing home industry and working hard to streamline the process of keeping track of referrals. With us, you can be sure that none of your referrals are falling through the cracks.

When it comes to tracking referrals, ease of use is our priority. On the single residents page, you will have the option to change the status of your referrals by simply clicking the relevant button above the patient's name. Status changes are linear and follow the order of new referral, in review, accepted, pending admission, and finally admitted.

  1. New Referral

    New Referrals are those that have just come in and require your attention as soon as possible. At this stage, they have not yet been reviewed by your team.

  2. In Review

    Referrals marked as In Review have been looked at by your team. The final determination has not yet been made.

  3. Accepted

    Referrals that your team has decided to accept into your facility. Now you will have to wait to see if the patient accepts the offer in return.

  4. Pending Admission

    The patient has agreed to enter the facility.

  5. Admitted

    The patient is now officially in the doors of your facility!

  6. Declined Referrals

    In the case where things don't go as planned, you always have the option to decline a referral. Simply click the thumbs down button and provide the reason for the decline so that we can prioritize data driven referral management.

Undoing a Status Change

If you ever make a mistake when applying a status, simply click the "X" next to the applied status and the status will revert back to the original state!

Filtering Referrals by Status

The above mentioned statuses will show up on the index table, showing you which referrals are at which status point. You can choose to filter out your index page to view just the referrals you need!

Keeping Track of Status Changes

To view any status changes, simply check out each resident's chat sidebar. You will see the status change, who changed it and the exact time the status was changed!
