The Clubworx Waivers feature makes it super simple to get your members signed up and ready to go. You can quickly create digital waivers, send them out in just a few clicks, and have everything stored safely in one place. New members can complete their forms online before they even walk through the door, , fast, and stress-free for everyone.

Waiver Settings
Determine what type of contact you'd like your waiver to update or create. Choose from Member, Prospect of Non-Attending Contact.
Choose if your waiver should be signed by All contacts, Individuals or Parents/Guardians with Dependants.
Determine if you'd like to restrict the minimum age of the primary signee. This prevents minors from being able to sign the waiver.
Select whether you'd like the waiver to undergo the approval process. Choosing to review and approve your waivers prevents duplicate contacts from being created.
Business Information
Enter your business information, including your address, and upload your logo. This information will appear at the top of the waiver when your contacts fill it out.
If you’d like your business details to populate automatically for all waivers, add them to your Clubworx account under Settings > Account > Profile Contact Information. You can upload your logo by navigating to Settings > Website > Website Design.
Contact Information
Rename option allows you to change the title of this section to suit your preference.
Manage required fields allows you to disable fields, or mark them as required. In this case 'Required' means that the member must fill out that field to be able to submit the waiver.
The contact can enter their details in this section. If the contact is a parent signing up a child, they have the option to add multiple dependents when filling it out.
The Emergency contact section can be turned on and off via the toggle on the right hand side. This section can also be turned into a 'required' section via an additional toggle on the right.
Questionnaire
The Questionnaires section allows you to add a customizable set of questions to your waiver form. This enables you to collect additional information from each person signing the waiver. This section can be turned on or off via the toggle at the right hand side. You are able to include multiple questionnaires per person on the one waiver. Examples of information collected via the questionnaire health history, training history marketing information and demographic information.
Title of Questionnaire – You can set a name for the questionnaire that will appear on the form.
Questionnaire Description – Optional field where you can add context or instructions for the signees.
Who should this be completed by/apply to?
Options allow you to specify which contacts have waiver questionnaires attributed to them.
This ensures that if multiple people sign a waiver, each provides their own set of answers.
Copy answers to contact record
Choose Yes or No to determine whether questionnaire responses are stored in each contact's record for future reporting.
You can add one or multiple questions. Each question includes:
Type (Text, Multiple Choice, Dropdown, etc.).
Question Title (the text that will be displayed).
Options to mark the question as Enabled or Required.
Terms and Conditions and Waiver and Release
The Terms and Conditions and Waiver and Release sections of your waiver can be easily enabled or disabled using the toggles on the right-hand side. You can also rename these sections to match your preferred titles.
Both sections are text boxes where you can include the fine print for your members to read and agree to.
Terms and Conditions typically cover the membership agreement, including details such as cancellation and suspension policies, insurance requirements, and registration information.
Waiver and Release is designed to outline liability and insurance-related information, ensuring members acknowledge and accept associated risks.
Membership Options
The Membership section of the Clubworx waiver is the part of the waiver where you display available membership options to your new members or signees and allow them to select one at the time of signing.
Key Features
Membership List: Shows all available membership options (e.g., Unlimited Classes Monthly Payment, 10 Class Pass, Personal Training sessions).
Payment Details: Displays the cost structure for each membership, including upfront fees and recurring payments.
Purchase Limits: Indicates if there are restrictions on how many times a membership can be purchased (in this example, it shows "N/A" for unlimited).
Show on Waiver Toggle: Lets you choose which memberships are visible for selection when the waiver is being signed.
Default Choice Toggle: Allows you to pre-select a membership as the default option for the signee.
Membership Settings
At the bottom of the section, you can configure how memberships are assigned when families sign a waiver:
Group Membership Plans by Category: Combine similar plans under one heading for a cleaner display.
Family Sign-Up Options: Choose how memberships are assigned for families:
Assign a paying membership to the parent only and free memberships to dependents.
Assign a paying membership to all signees.
Allow each signee to choose their own membership (or opt out entirely).
Payment Authority
The payment authority section will appear if you have the appropriate payment processor. This section allows you to collect credit card or bank account details from your members. This section can be turned off and on from the toggle on the right hand side.
Payment Settings allow you to share wallet details between family members. You can also restrict payment options to either bank account or credit card, or set the preference for which option appears on the waiver for your members to select.
Signature and Witness Details
This section allows you to request a signature and witnessing signature from the signee.
Electronic Consent
An electronic consent clause is the section within the waiver that specifies that the signer’s acknowledgment, agreement, or signature provided electronically is legally valid and binding — just like a traditional handwritten signature.
Waiver Communications Content
Completed Waiver Page Content: This is the page that appears when the contact submits their waiver. Use the option on the right hand side to customise the copy on this page and add a call to action button.
Completed Waiver Received Email: This email can be sent to the contact when the waiver is submitted as an email received. Use the customise option to turn this off or on, and customise the text of the email.
Pre-filled Waiver Invite Email: This is the email that is sent with the waiver link included when the waiver is sent from an Automation, or from the contact's profile as a pre-filled waiver. Use the customise option to turn this off or on, and customise the text of the email.
Approved Waiver Receipt email: This email is sent to the contact when their waiver is approved and includes a copy of the signed waiver. Use the customise option to turn this off or on, and customise the text of the email.
