Waiver questionnaires allow you to create custom questions to collect important information from your members. It is now possible to have more than one questionnaire on your waiver. This means:
You can include multiple versions of the same questionnaire to collect information from each contact included in the waiver.
You can include more than one Questionnaire on the waiver to collect different types of information in different sections.
Creating your Questionnaires
To create your questionnaires, create or edit your waiver and go to the Questionnaire Section.
Give your Questionnaire a name. Add a description as an introduction to your questionnaire if you choose to.
3. Select who you would like to fill out the Questionnaire.
Each individual signee - allows every contact on the waiver to have a questionnaire attached to them.
Primary signees only - Assigns the Questionnaire to just the primary signee.
Dependent signees only - Assigns the Questionnaire to just the dependents.
Attending Signees only - Assigns the Questionnaire to just contacts who will be participating in sessions.
Non-attending signees only - Assigns the questionnaire to just the contacts who are not attending.
4. Choose if you would like to copy the answers to the contact record. Copying the answers to the contact record allows you to report on these answers via custom reports.
5. Create your questionnaire questions. There are 3 types of questions that you can set up within your questionnaire. With every question you set up, you have the option to set it as a required question or not. You can also disable the question if you no longer require it.
Text: This is an open-ended type of question where a contact is are required to key in an answer. For example, "Have you trained Jiujitsu previously?"
Checkbox: A contact can check or uncheck a box to answer the question
Select: This option allows you to set up a question with multiple answers as options. A contact can then select an answer from a dropdown list.
For example:
Question: "Have you trained Jiujitsu previously?"
Answer option 1 : Yes
Answer option 2 : No
6. You can add additional Questionnaires by selecting the + Add another Questionnaire option. This appears at the bottom of the Questionnaire section.
7. Use the options on the right hand side of the Questionnaire to disabled the questionnaire, change the order of the questionnaires, duplicate the questionnaire or delete the questionnaire.
Completing the Questionnaire
If the questionnaire has been set up to be signed by multiple contacts, each contact will see the questionnaire appear below their contact information on the waiver.
To complete the questionnaire the member should click on the start button on the right hand side. They will then see the questions and be able to submit their answers. Once they have completed the questionnaire it will show as completed below their contact details.
