Intake forms in CoachRx let you collect important information from clients when they first join your roster. You can customize the questions, send forms to new clients, and review their responses - all from your coach dashboard. This article covers how to set up and manage intake forms.
What Are Intake Forms
Intake forms are questionnaires you send to new clients to gather baseline information before you begin programming for them. Common intake form topics include health history, injury history, training background, goals, nutrition habits, lifestyle details, and availability.
Clients receive and complete intake forms through the CoachRx mobile app. Their responses are stored on their client profile in your coach dashboard.
Creating an Intake Form
Navigate to your Settings or Forms section in the CoachRx dashboard
Select Intake Forms
Click Create New Form (or edit an existing template)
Add your questions using the available field types (text, multiple choice, scale, etc.)
Arrange questions in the order you want clients to see them
Save your form
You can create multiple intake form templates for different client types or onboarding workflows.
Sending an Intake Form to a Client
Intake forms can be sent to clients as part of the onboarding process:
Go to the client's profile
Navigate to the Assessments or Forms section
Select the intake form you want to send
Assign it to the client
The client will receive a notification in their CoachRx app to complete the form. You can also configure intake forms to be sent automatically when a new client is added to your roster.
Viewing Completed Intake Forms
Once a client completes their intake form:
Go to the client's profile
Navigate to the Assessments or Forms section
Click on the completed intake form to view their responses
Responses are stored on the client's profile and can be reviewed at any time.
Re-sending an Intake Form
If a client needs to fill out an intake form again (for example, during a re-assessment or if their original responses need updating), you can re-send the form by assigning it to them again from their profile. The new submission will be stored alongside any previous ones.
Editing Your Intake Form Templates
You can edit your intake form templates at any time. Changes to a template will apply to future sends but will not retroactively change forms that have already been sent or completed.
To edit a template:
Go to Settings or Forms
Select the template you want to edit
Make your changes
Save
Tips for Effective Intake Forms
Keep forms focused - ask what you need to start programming, not everything at once
Use a mix of question types (open-ended for goals and history, multiple choice for quick selections)
Review responses before the client's first consultation so you can come prepared
Consider creating different templates for different client types (e.g., new to training vs. experienced athletes)
