The "Follow-up" module allows you to schedule the sending of a questionnaire via e-mail or SMS or to set the appropriate time for an assisted follow-up with the patient. The pharmacist then analyzes these follow-ups on the dashboard.
If you have access to the "Follow-up" module, the "Follow-ups ▼" option will appear at the top left of your screen after the name of your facility.
Plan a follow-up
To schedule a follow-up, click on the "Follow-ups" tab, then on "Plan a follow-up" to be redirected to the appropriate page.
Follow-up information
Start by selecting the follow-up questionnaire you wish to schedule for your patient. Enter the desired keywords in the search bar, then select the appropriate follow-up for the patient. If the reason for follow-up is missing, you can select "Generic follow-up:" followed by the keywords you've entered in the search bar.
Once you have selected your follow-up reason, you can customize your follow-up with the following elements:
Follow-up method: Choose whether the follow-up will be sent to the patient or whether you wish to be notified to contact the patient at the time of follow-up;
Follow-up period: You can choose from three types of follow-up:
Single follow-up, which will be sent once after the selected number of days;
Recurrent follow-up, with the first follow-up sent after the selected number of days, then at each selected interval (Recurrence period). You can choose when the recurrence will end (Never, on a specific date or after a certain number of sendings);
Immediate follow-up, for which follow-up will either be sent immediately or started immediately, depending on the chosen follow-up method.
Customize follow-up: Depending on the questionnaire, you can add details to your follow-up. When you check the "Add precisions" box, you can select additional personalization elements, such as the medication according to the condition;
Additional information: You can add the following to your follow-up:
Internal notes for the pharmacist who will analyze the follow-up (these notes are not visible to the patient);
Personalized message to the patient, displayed before the first question in the questionnaire;
Personalized questions for the patient to answer at the end of the follow-up using a text box.
Patient information
In this second step, select the patient you want to send a follow-up to.
If this patient has never had any consultations or follow-ups with the Empego platform, please create a profile for him/her by clicking on "Create new patient."
If the patient already has an Empego profile, you can search for them using the search bar, select them by clicking their name, and click the "Next" button.
A patient can be found using his last name, first name, RAMQ or telephone number. If several choices are available, make sure that a second identifier (e.g. date of birth, if applicable, telephone or e-mail) allows you to confirm that the selected profile is the right one.
Confirmation
In the third step, you can validate the selected information and details and confirm your follow-up schedule. Suppose your follow-up is immediate and assisted; when you press the confirmation button, the follow-up questionnaire will automatically open, allowing you to answer the questions with the patient.
If you answer the questionnaire with the patient on your computer, ask the questions as they appear on the screen. Questions marked with a red asterisk are mandatory; an answer must be selected to move on to the next question. If a question is not mandatory, it can be skipped by clicking the "Next" button at the bottom of the screen. To go back if necessary, click "Previous" and then "Next" to return to the next question.
