Your immunization record shows which vaccinations you've had. We ask for proof of MMR, Hepatitis B, TB and Varicella, plus your COVID-19 vaccination.
If you don't already have a copy, your family doctor can usually print or email one for you — just ask their office for your immunization record.
If you don't have a family doctor, you still have options. A walk-in clinic can often print your record or arrange the vaccinations you're missing. Your local public health unit may hold your childhood immunization records. If you were vaccinated at a pharmacy, they can usually reprint the receipt or record. And if you studied or worked in healthcare before, your school or previous employer's occupational health team may have a copy on file.
