Physical Gift Card Request (CountyCare members only)

Criteria for Physical Cards:

  • They will only be given to County Care members.

AND

  • They must be either legally blind in both eyes or illiterate members. Illiterate means can not read or write in the language that they speak

    1. EX: Spanish speaking and cannot read or write in Spanish.

Non-qualifying criteria:

  • The member can change language on website or device to match their language.

  • They are NOT a CountyCare member

  • Tech illiterate or tech challenged

  • Not having tech does not qualify.

  • Not having internet

  • Not wanting to use Fresh Funds on Instacart

  • Member has a Foodsmart.com account profile issue. Please look in the FNN under profile issue for next steps.

  • Instacart deactivated the member’s account. Please look in the FNN under deactivate for how to help your member.

  • If they have a family member, neighbor, friend or RD that can help them with the redemption and procurement process, they are not eligible.

  • Members CANNOT request a physical card. A medical professional has to request one based on their professional determination of a diagnosis.

CountyCare only allows for them to be given in extreme cases, as CCH does not want us to give any.

Walmart physical cards may take up to 21 business days to receive via mail once it is processed. Holidays can extend that timeline.

Support does not reach out to members to verify eligibility, as they are not medical professionals and cannot take on the liability of a medical professional. If more information is needed, the Incentive Team will reach out to the RD inside the ticket. Once the determination is made per the above criteria, the RD will be messaged if the member was not eligible or if the physical card was sent to the member.

All Physical Cards must be requested through a Physical Card ticket below, then the incentive team will send out their physical card. Please only request after every 3 visits, due to fees incurred.

If you see that the member is coming up on the end of their eligible visits for the year, please turn in all visit dates at one time, as fees easily outweigh the incentive if only one incentive is sent to the member.

Please submit a ticket if your member is eligible for a physical gift card.
