Initial Notification or Register of Injury

The Initial Notification or Register of Injury form is available for relevant injuries based on the Policy/State selected when being entered into the system.

The process below is the same for both the Initial Notification and the Register of Injury, with the screenshots showing the Initial Notification process.


ADDING INITIAL NOTIFICATION OR REGISTER OF INJURY

The form can be added from the Summary tab of the claim file in two ways:

Select Initial Notification from the Add Information drop-down menu or click on the red Initial Notification button in the Claim Progress section.


COMPLETING THE INITIAL NOTIFICATION OR REGISTER OF INJURY

Users can either 'Complete' the Initial Notification in-system, or 'Upload' an already completed Initial Notification.

Complete Initial Notification

Many fields will be pre-populated based on information already within the Injury record. Scroll through the remainder of the form and fill in the fields as required. From here, select either Save or Save & Email if you need to email the form from the system.

Upload Initial Notification

If the Initial Notification has been completed outside of Solv, this can be uploaded to the system using the Upload option.

The date defaults to 'today', but this can be adjusted as needed. An attachment will be mandatory when using the Upload option.

The iCare Initial Notification Integration will not work when using the 'Upload' option for the completion of the Initial Notification. For this to work, you need to 'Complete' the form in-system.


VIEWING THE INITIAL NOTIFICATION OR REGISTER OF INJURY

The Initial Notification form will be stored in the Claim Documents section of the claim file, found under the Claim Management menu.

To view the details of Initial Notification, select the arrow button to the left of the date.

The three dots to the right of the record row will give you the option to edit, email or delete the record, and the upload icon allows you to upload an attachment directly against that record. If an attachment is already uploaded, that icon will change to a paperclip.
