Add & Manage a Client Care Plan using Forms

This lets you generate a care plan entry from a selected form, store the form values with the care plan, and preview the form snapshot on the care plan card.

Care plans created from forms can be edited later. When you edit and submit again, the existing care plan is updated.

How to add a care plan via form

  1. Open the client’s Care Plan tab.

  2. Select Use Form.

  3. In the picker titled Add care plan via form, select the form you want to use.

  4. Fill out the form.

  5. Submit the form.

What you will see after submission

  • A new care plan card appears.

  • Form-based care plans show a Form badge.

  • The card shows:

    • Created by:

    • Created:

    • Renewal:

  • If the care plan was created from a form, the card can show a form snapshot preview.

Edit and update a care plan created from a form

This is supported.

  1. Find the care plan card with the Form badge.

  2. Open the card actions menu.

  3. Select Edit.

  4. Update the form values.

  5. Submit again.

When you submit an edited care plan form, Imploy saves it as an update. You will see Care plan updated successfully.

Fullscreen fill and sharing a fill link

When filling a care plan form in fullscreen, a share option is available for editable fills. It copies a fill link and shows Fill link copied when successful.

What happens next

  • New submissions create a care plan entry.

  • Editing an existing form-based care plan updates the same care plan entry rather than creating a new one.
