Treatment plans in Klarify

Add a client's treatment plan to Klarify, edit it in the app, and have Klara draft or update one for you.

You can keep a treatment plan in Klarify for a client, a couple, or any other group. Once it's on file, Klarify reads it every time it writes a note for that client or group. Your notes line up with the goals you already set, instead of starting from scratch each session.

You do not need to rewrite your plan. Upload the one you already have, or have Klara draft one. Either way, you can edit it right inside Klarify.

Add a plan you already have

From the Treatment Plan tab

  1. Open the client from Clients.

  2. Select the Treatment Plan tab.

  3. Press Upload existing and choose the file.

From the client's Files

  1. Open the client, then go to Files.

  2. Upload the file.

  3. Set Category to Treatment Plan.

That last step matters. If the file is uncategorized, Klarify will not treat it as a treatment plan.

From inside a session

  1. Open the session and go to the Klara panel.

  2. Click Treatment plan.

  3. If the client has no plan on file yet, an upload box opens. Drop the file in.

From a file you already uploaded

If the plan is already in your library, open the client's Files tab, use Link from library, then set the category to Treatment Plan.

Klarify accepts PDF, Word (.docx), RTF, HTML, Markdown, and plain text files.

Edit a plan in Klarify

You don't need to download a plan, edit it in Word, and upload it again. Edit it in place:

  1. Open the client and select the Treatment Plan tab.

  2. Press Open on the plan.

  3. Press Edit and make your changes. They save automatically as you type.

  4. Press Save when you're done to lock the plan again.

Want Klara to make the changes? With the plan open, press Edit with Klara and tell her what to change. Or, on the Treatment Plan tab, press Update with Klara and she'll update the plan with the client's latest sessions.

You can also update a plan from a session. Open the session's Treatment Plan tab. If the session is newer than the plan, Klara offers to Update treatment plan based on this session.

Admins who view another therapist's client see that client's plan as read-only.

What happens next

Once a plan is on file for a client, Klarify uses it automatically. There is no extra button to press.

Every note Klarify writes for that client is generated with the plan in front of it, alongside the session transcript. So the note is written against the goals, modality, and interventions you already documented.

This works for recorded sessions, uploaded audio, and notes you regenerate.

A few details worth knowing:

  • Klarify reads the three most recently updated plans on file for that client.

  • It reads about the first 1,200 words of each plan. Very long plans get shortened.

  • The session has to be linked to a client or a group. Sessions with nobody attached will not pull a plan.

  • Klarify reads the plan at the moment the note is written. If you update the plan and want an older note to reflect it, regenerate that note.

Plans for couples and groups

A group can have its own treatment plan. Open the group from Clients and select the Treatment Plan tab. From there you can Upload existing to add the plan you already have, or Create with Klara to have Klara draft one from the group's sessions. Open and edit a group plan the same way as a client plan.

Group sessions also show the Treatment Plan tab in the session overview. Klara reads the group's session notes when it drafts or updates a group plan, and every note for that group is written with the plan in front of it, the same as for an individual client.

Use a plan in a chat with Klara

Sometimes you want the plan in the conversation, not just in the note.

In the note view Klara panel, click Treatment plan and pick the plan.

Klara can now read it for that chat. This is useful for questions like:

  • "Are we still tracking against her goals, or has the focus shifted?"

  • "What should I prioritize next session, given the plan?"

Click Treatment plan again to remove it from the chat.

Have Klara draft a plan

If the client doesn't have a plan yet, Klara can write one.

  1. Open the client and select the Treatment Plan tab.

  2. Press Create with Klara.

  3. Give the plan a title. Optionally add an example template, pick the sessions to base it on, and add instructions.

  4. Press Create with Klara. The plan opens and Klara drafts it as you watch.

  5. Review it and edit anything you want to change.

The draft is saved as the client's treatment plan, so your notes start using it right away. You don't need to download it or upload it again. Each client has one plan, so Create with Klara only shows when the client doesn't have one yet. To change an existing plan, use Update with Klara or Edit.

Useful instructions include:

  • "Build a trauma-informed treatment plan using EMDR based on these notes."

  • "Draft a relapse prevention plan for a client in early recovery."

  • "What treatment objectives fit a couple dealing with infidelity?"

You can also start from a session: open the session's Treatment Plan tab, and Klara drafts the plan right there from that session's note.

Desired outcomes

On the client's profile you'll also find Desired outcomes. These are the active goals for that client, and they get pulled into note writing the same way the plan does.

You can add or edit them by hand at any time. They're a good lightweight option if you want goal tracking without a full plan document.

What Klarify does not do yet

  • There's no structured plan builder with goal and objective checkboxes. A treatment plan in Klarify is a document you upload or have Klara draft, and you can edit it in the app.

  • Klarify does not track progress against plan objectives automatically.

  • The plan has to be uploaded under your account for that client or group. A plan another therapist uploaded for their own client record will not appear on yours.

If your plan isn't showing up in notes

Check these, in order:

  1. Is the file categorized as Treatment Plan? This is the most common cause.

  2. Is the file linked to that client, not just sitting in your library?

  3. Was the note generated before you uploaded the plan? Regenerate it.
