JHC Overtime & Callback Forms — Updated Template (Entry Groups)

How to use the updated Overtime and Callback form templates with entry groups in Med App.

The JHC Overtime and Callback form templates have been updated. The new templates use entry groups, which means you can now log multiple overtime periods or callbacks within a single form rather than starting a new form each time.

This makes it easier to keep track of your entries across a pay period — just keep adding entries, save your draft, and submit once you're ready.

What's changed?

One form per overtime period or callback with empty question sets

How To Use

  1. Open Med App and tap the Forms tile on the main guide page.

  2. Select the updated Overtime or Callback form template (the old ones won't be visible), then tap Start New Form.

  3. Fill in your general information (Employee Number, Position, Department).

  4. Tap + Add Entry to add an overtime period or callback entry. Each entry captures the date, start time, finish time, and details.

  5. To add more entries, tap + Add Entry again. You can add up to 50 entries per form.

  6. Tap Save Draft at any time to save your progress. You can come back and add more entries later.

  7. When you're ready (typically at the end of the pay period), sign the Employee Declaration and tap Submit.

Tips

  • Pre-fill Employee number - if you want your employee number to be pre-filled everytime, tap 'more' then 'my IDs', tap the pencil button next to employee number. Once you have added it, it will automatically add it to the overtime or callback form when you start a new one.

  • Update the form title to reflect the pay period by tapping the pencil icon at the top of the form. This helps you identify the period at a glance.

  • One form per pay period. Aim to submit one form per pay period..

  • Submit on time

  • Seconded Drs — please include your HE Number in the comments section.

  • After submission, a completed copy is automatically sent to medicaladmin.jhc@ramsayhealth.com.au and is available for PDF export from Med App if you want a copy.
