When you'd use this
Sometimes you need to fill in a form on a computer rather than your phone, but you might not have access to your phone — for example, if you can't bring your phone on a particular shift. This guide covers two ways to open your form on a desktop browser.
Your form link stays live for as long as the form is assigned to you, so with a couple pre-steps you can set this up ahead of time so you can access it when you need it.
Option 1: Open it on your phone first, then email yourself the link
Do this while you still have your phone (for example, before your shift):
Open the form in the Med App.
Tap the desktop icon on the form. You'll be shown a 5-digit code, go to web.med.app/form and enter the code. The form will open in a desktop view.
Copy the full URL from the browser address bar.
Email that URL to yourself.
When you're at a computer, open the email and click the link to go straight to your form.
Option 2: Use your 5-digit code on the web
If you've made a note of your 5-digit code, you can open the form on any computer:
In a browser, go to web.med.app/form.
Enter your 5-digit code.
Your form opens, ready to complete or re-save as a draft.
Good to know
The code and link are unique to your copy of the form and stay valid while it's assigned to you.
If the form is retracted or reassigned, a new code and link are generated.
