Guide for preceptors on how to complete ANSAT forms via Med App

Med App allows hospitals to complete assessments digitally on a mobile device, tablet or computer to improve efficiency and reduce unnecessary paper-based waste. We'll provide an overview of the process and then progress into the more detailed steps to complete your assessment.

This workflow is specifically for the ANSAT for Transition to Practice Nurses and Preceptors.

Completing an assessment as a Preceptor (via Med App)

When a transition-to-practice nurse selects you as their preceptor, follow these steps to complete the ANSAT form:

  1. You'll receive an SMS

  2. Click the link to start the assessment now. Alternatively, to access the form later, Open Med App > Click 'More' (bottom right corner of the app) > Click 'My Forms'

  3. Confirm your identity (You are only asked this the first time)

  4. Complete the assessment on your mobile or click on the computer icon on the top right of the screen to complete the form via computer (See how)

  5. Once the form is completed you can:

    1. Save as a draft

      or

    2. Send the completed form to the Transition to Practice nurse for them to review and sign.

Completing an assessment as a Preceptor (via email)

When a transition-to-practice nurse selects you as their preceptor, follow these steps to complete the ANSAT form:

  1. You'll receive an email link to the ANSAT form

  2. Click the link to start the assessment. If you wish to save the draft, click save. You can access the same form via the same link at a later stage.

  3. Once the form is complete and ready for submission, press continue to send the form back to the Graduate Nurse for their review. NOTE: This should only occur after a face-to-face discussion has occurred.

Retracting an assessment: This can be done at the time of submission by pressing 'retract & resubmit' or after you have completed the process (see how)

Using a computer or tablet device to complete your assessment

It's easy to switch to another device to complete an assessment, all you need is access to a web browser. See the following steps:

  1. During an assessment, press the desktop icon in the top right corner of any assessment

  2. Open a web browser on any device and enter web.med.app/form

  3. Enter the unique code from your mobile device

  4. Complete & submit the assessment
