How to complete a Mid or End-of-Term Assessment as a junior doctor, clinical supervisor or term supervisor digitally using Med App

Med App allows hospitals to complete assessments digitally on a mobile device, tablet or computer to improve efficiency and reduce unnecessary paper-based waste. We'll provide an overview of the process and then progress into the more detailed steps to complete your assessment.

This workflow can be used for several assessment types:

  • Mid Term Assessments

  • End-of-Term Assessments

  • Prevocational doctor self-Assessments

Starting an assessment as a prevocational doctor

After you receive a notification about an assessment, it will be available on Med App:

  1. Go to Med App > More > My Forms

  2. Select a Form that is 'Assigned to you'

  3. Complete form section

  4. Enter your supervisor's name and preferred contact (either SMS or Email) & Submit

  5. Alternatively, 'save as draft' to resume later

Form progress & Status: You'll be able to see the status of your assessment to see where it is in the process by opening My Forms.

Completing an assessment as a Clinical Supervisor

When a prevocational doctor selects you as their supervisor, follow these steps:

  1. You'll receive an SMS

  2. Click the link to start the assessment

  3. Confirm your identity (You are only asked this the first time)

  4. Complete assessment

  5. Complete the assessment on your mobile or click on the computer icon on the top right of the screen to change devices

  6. Submit the form to the Term Supervisor for approval

Clinical supervisors who are also Term supervisors:

Select your role as term supervisor if you are both the clinical & term supervisor to avoid sending out the form twice.

Retracting an assessment: This can be done at the time of submission by pressing 'retract & resubmit' or after you have completed the process (see how)

Completing an assessment as a Term Supervisor

As the Term Supervisor, you will need to complete the approval process:

  1. You'll receive an SMS

  2. Click the link

  3. Review the assessment on your mobile or click on the computer icon on the top right of the screen to change devices

  4. Draw your signature and Submit it back to the prevocational doctor for final review and signature

Retracting an assessment

If you disagree with the assessment you will need to ask the assessor to retract the form and make the changes.

Completing the assessment as a prevocational doctor

Once the Term Supervisor has completed their section the form will be available to the intern:

  1. Go to Med App > More > My Forms

  2. Select a Form that is 'Ready to sign'

  3. Review the form

  4. Draw your signature

  5. Submit or save as a draft to complete later

  6. Once Submitted you can Export the assessment as a PDF

Using a computer or tablet device to complete your assessment

It's easy to switch to another device to complete an assessment, all you need is access to a web browser. See the following steps:

  1. During an assessment, press the desktop icon in the top right corner of any assessment

  2. Open a web browser on any device and enter web.med.app/form

  3. Enter the unique code from your mobile device

  4. Complete & submit the assessment

Retracting an assessment as a Med App user

If you are using Med App to complete assessments, use the below method to retract and resubmit your forms or assessments

  1. Open Med App

  2. Locate 'More' in the bottom navigation > My Forms

  3. Open a form/assessment with the 'assigned' progress status

  4. Press 'Retract and Resubmit'

  5. Edit/confirm the details of the previously completed form

  6. Enter the new assignee or assessor details & submit

Retracting an assessment as a non Med App user

A clinical supervisor can retract an assessment in 2 ways:

  1. At the time of submitting an assessment by pressing 'retract & resubmit'.

  2. After you have submitted the assessment.

    1. Note: The person retracting will only need to edit their response not rewrite it.

Retracting an assessment after you've submitted it

  1. Locate the SMS from Med App

  2. Open the link for the correct clinician & assessment name

  3. You will need to go through the steps; Review, complete, and sign steps before seeing the 'Retract' Button

  4. You can then enter the assessment details, sign and submit to the clinical supervisor

Exporting assessments & forms

Exporting your assessment as a PDF from your phone is easy!

  1. Go to 'More' in the bottom right of your nav bar at the bottom of your phone

  2. Toggle tabs to All Forms to find your completed forms

  3. Select a completed form

  4. Press 'Export as PDF' to download your completed assessment
