How to Complete a Workplace-Based Assessment (WBA) - Clinician Guide

Complete digital assessments on your mobile, desktop or any web based device using Med App 'Forms'

Med App allows hospitals to complete assessments digitally on a mobile device, tablet or computer to improve efficiency, and completion rates and reduce unnecessary paper-based waste.

Below is a summary of how it works or you can skip to the following sections

Receiving assessment notifications

If you have push notifications disabled for Med App, you will receive an SMS instead.

Step-by-step guide on completing assessments

Step 1: Completing a form as a clinician

After your hospital administrator has sent you a form, it will be available on Med App. You can then follow these steps:

  1. Go to Med App > More > My Forms

  2. Select form that is "Ready to Submit"

  3. Complete form section

  4. Select your assessor

  5. Submit, can share QR code

  6. You'll be able to see status changes to 'awaiting assessor submission'

  7. Assessor should now be able to complete their section of the form

Step 2: Completing a form as an Assessor

If a clinician has selected you as their assessor the form will be available on Med App. You can then follow these steps:

  1. Go to Med App > More > My Approvals

  2. Select form that is "Ready to Submit"

  3. Review clinician form section

  4. Complete your assessor section

  5. Sign your section

  6. Submit, can share QR code with the clinician for them to review and sign

Step 2 Alt: Using computers and tablets to complete an assessment

It's easy to switch to another device to complete an assessment, all you need is access to a web browser. See the following steps:

  1. During an assessment, press the desktop icon in the top right corner of any assessment

  2. Open a web browser on any device and enter web.med.app/form

  3. Enter the unique code from your mobile device

  4. Complete & submit the assessment

Step 3: Final sign-off by clinician and submission to your administrator

After your assessor has completed, signed and submitted their form, it will be available on Med App for you to sign off. You can then follow these steps:

  1. Go to 'My Forms' and see the form "Ready to Sign"

  2. Open form and review all completed sections

  3. Continue to add your signature and submit

  4. The form is now completed and will be available to your administrator to review

  5. Your administrator will now be able to view the completed form

Exporting your assessment as a PDF

  1. Go to 'More' in the bottom right of your nav bar at the bottom of your phone

  2. Toggle tabs to All Forms to find your completed forms

  3. Select a completed form

  4. Press 'Export as PDF' to download your completed assessment

FAQs

Q: How do I know when I have a form ready to complete?

A: If you have push notifications enabled for Med App, you will receive a push notification to notify you when a new form is available, and when it's coming up to the due date. If notifications are disabled you will receive an SMS.

Q: Can I complete a form on a desktop device?

A: Yes, in the top right hand corner of the form there is a a desktop icon. When you tap on this icon you will see instructions to access https://web.med.app/form, you will need to enter the unique code on this page to access the form.

Q: Can I fill out an assessment without being signed up to Med App?

A: Yes you can. If you have an assigned form but not registered, you will be emailed a link to complete your form or assessment.

Q: Can I submit a form if my device is offline?

A: No, currently you will need an internet connection to submit a form. If you are in an area with an inconsistent connection, you can save a draft version of the form while you are connected so that you can ensure your changes are not lost.

Q: I can't find my assessor from the list of available assessors

A: Please contact your administrator to let them know that your assessor is missing and they should be able to resolve this issue.

Q: What happens if I don't complete the form by the close date?

A: Your administrator should allow ample time to complete a form prior to the close date. You cannot submit the form after the close date and you will need to contact your administrator to make a form available.
