Understand Audit Agent results, pass rates, and verdicts

How to read Audit Agent dashboards, criteria results, session results, and clinician comparisons.

Audit statuses

An audit may show as queued, running, processing, completed, failed, or cancelled.

If an audit is queued or running, wait for processing to finish. The audit dashboard refreshes while jobs are active.

Pass rate

Pass rate depends on the scoring strategy used by the template.

For verdict-based scoring, pass rate is based on sessions rated strong or adequate.

For points-based scoring, pass, needs review, and fail depend on the template's percentage thresholds.

Verdict labels

Verdict-based audits can show:

  • Strong.

  • Adequate.

  • Weak.

  • Missing.

Points-based audits may display equivalent outcome labels such as pass, needs review, fail, and missing.

Criteria view

The criteria view shows how each criterion performed across the audited sessions. Use it to identify the biggest documentation gaps.

Clinician view

Where enabled and where multiple clinicians are included, Audit Agent can compare clinicians across criteria. Use this for governance and training patterns, not as a standalone judgement of individual clinical competence.

Session view

The session view shows individual session-level results. Use it to inspect why a specific session passed, failed, or needs review.

Executive summary

The executive summary highlights issues requiring attention, especially criteria below threshold and higher-priority criteria.

Export

Completed audit results can be exported to CSV from the audit result page where available.

If results look wrong

  • Check the selected template and criteria.

  • Check the selected source material.

  • Check whether sessions were skipped because the selected note type was missing.

  • Open session-level results to inspect the evidence.

  • Rerun with a narrower date range or clearer criteria if needed.
