Overview of Custom Form Fields in Edlio Pay
When collecting payments for school events, bookkeepers often need to capture critical student information alongside payment—such as emergency phone numbers, dietary restrictions, student ID verification, or digital acknowledgments. In Edlio Payments (OSP), the Form Fields tab provides a flexible questionnaire builder attached directly to individual items.
💡 Best-Practice Adoption Tip: For field trip permission slips or event waivers, combine a String field (labeled Parent_Full_Legal_Name) with an agreement List dropdown (options: Yes, I give permission / No) and set both to Required. This captures a legally verifiable digital consent record linked directly to the transaction receipt.
Supported Field Types
Choose from three native field types when configuring questions:
String: Single-line open text input for short answers, names, phone numbers, or notes (up to 255 characters).
Date: Interactive calendar date picker that enforces valid date entry formats (ideal for birthdates, medical clearance dates, or event dates).
List: Dropdown picklist containing predefined options that you define, separated by commas or line breaks. Prevents spelling errors and standardizes reporting.
Step-by-Step: Adding Form Fields to an Item
From the Pay Management Dashboard, click Products and open your desired item in the editor.
Click the Form Fields tab.
Click Add Field.
In the Field Label box, type a clear, unique question prompt (e.g.,

Check the Required box to ensure purchasers cannot complete checkout without providing an answer.
Click Save at the bottom of the page.
