How to Create Form Templates

In order to capture game details exactly as needed by your organization, custom game templates can be created and formatted as an addendum (i.e., Location/Field Conditions or Sportsmanship Info) to the existing Post-Game and Referee Game Reports. These templates can then be added to the Post-Match Report to ensure the right details are being captured based on play type or competition rules.

To create an addendum game report, follow the steps below:

  1. From the navigation, select Settings > Forms & Docs > Web Forms. On the Web Forms tab, click on the mini menu and select Add New Template.

  2. Add a title for your game report and leave the Form Type drop-down to its default blank selection. Selecting Games will pull in game-oriented fields that already exist on the default Post-Game Reporting forms, so selecting Games is not necessary at this time. Once complete, click Add.

  3. You can now begin creating your custom game report. Click the mini menu to edit the section with options that include:

    • Editing the section title

    • Removing the section entirely

    • Adding new items to the section. Adding items will include standard options of text inputs, multiple choice, checkboxes, or memo text.

  4. In order to use the template on existing game reports:

    • Navigate to either the Post Game Report or Referee Game Report and click Add Section.

    • Choose From Template to add an existing template. A drop-down menu will display and list your saved templates. Select your template and click Add. Templates can still be edited as necessary on your new form. Click Add once complete.
