Forms let you gather information, and optionally payments, from your members. This guide walks you through building one from scratch, from naming it to publishing it live.
Start your form
From the Forms tab, tap + Create Form.
Enter a Form Name (required) and a Form Description (optional).
Tap Continue. Your form is created in Draft status, and you'll land on a summary screen with 3 sections: Add Questions, Form Settings, and Collect Fees.
Note: Form Name is required. If you leave it blank, you'll see "Form Name is required." and Continue stays disabled.
Add your questions
Tap Add Questions, then + Add a Question.
Choose a question type: Short Answer, Paragraph, Multi-Select, Single Select, File Upload, Date, or Signature.
Enter the question text, plus options (for Multi-Select or Single Select) or instructions (for File Upload or Signature).
Toggle Required if members must answer this question.
Tap Add Question. It appears in a reorderable list, drag the handle to reorder, or use the edit and delete icons to change it.
Repeat for each question, then tap Save Questions • N Added.
Tip: A Signature question can include an attached document, like your terms and conditions, that members must scroll through before signing.
Keep in mind:
Multi-Select and Single Select questions need at least 2 options. Fewer than that and you'll see "At least 2 options are required."
Add Question stays disabled until you've entered question text.
Choose who can respond
Tap Form Settings, then Select the Groups who can Respond.
Choose All Members, or pick specific groups (including sub-groups, for example a "Juniors" group can contain "Junior Development Squad," "U15 Parents," and "U16 Parents").
Tap Share with N Groups.
Optionally, toggle Set an Expiry Date and pick a date. If you set one, the form automatically closes at the end of that day.
Tap Save.
Note: You must select at least one group before you can publish. If you try to save with no group selected, you'll see "Select at least one group to continue."
Collect a fee (optional)
If you want to charge members to respond, you can turn on fee collection:
Tap Collect Fees and toggle it ON.
Enter your Form Fee.
PLAI shows the club's net receipt after the processing fee is deducted. The checkout fee is paid by the member, on top of your form fee. Processing fees vary by region, so the exact amount shown is calculated for your club's location.
Select a Payout Destination (your connected account), then tap Save.
Important: If fee collection is turned on, both the Form Fee and a Payout Destination are required before you can publish. Leaving the fee empty shows "Enter a form fee." Leaving the payout account unselected shows "Select a payout account to collect fees."
Preview and publish
Tap Preview Form to see exactly what a member will see, labeled "Member preview." This view isn't interactive.
When you're happy with it, tap Publish Form.
A confirmation modal ("Ready to go?") shows how many members or groups will be notified, and reminds you the form is only editable until the first response comes in.
Confirm to publish. Your form's status changes to Open, and every eligible member is notified.
Important: Once someone submits a response, you can no longer edit the form's questions or fee settings. Group targeting and the expiry date can still be changed.
If you're not ready to publish, tap Save as a Draft at any point instead. Draft forms are stored under Drafts and aren't visible to members.
You'll also need at least 1 question and at least 1 selected group before you can publish. Trying to publish without either shows "Add at least one question before publishing." or "Select who can respond before publishing."
