✹ When You Need This Guide

  • You want to start a Care Plan for a patient

  • You’re ready to assign recurring visits for a specific appointment type

  • You want to ensure patients are on track with their treatment or wellness plan

Care Plans are a powerful tool to help patients stay consistent, organized, and on the path to progress—while making your team's day more structured!


✅ How to Create a Care Plan

  1. Go to the CONTACTS tab and select “Show Contacts”
    This will bring up your full contact list.

  2. Search for the patient
    Use the search bar in the top-right to find the patient you want to assign the Care Plan to.

  3. Click their name and select “Create Care Plan”
    You’ll now see a “Create Care Plan” button on their contact page.

  4. Choose the appointment type
    This is the type of visit the patient will be scheduled for throughout their plan.

  5. Select the provider
    Pick the doctor or practitioner that will see this patient.

  6. Click “Start Care Plan” to begin
    Now you can begin scheduling their visits.
    ​

  7. Set the appointment times and build out the schedule

    • Choose the time(s) for each day of the week

    • Click “Add Week” to create a repeating schedule

    • Adjust each week as needed
      ​

  8. Click “Start Care Plan” again to finalize
    Once finalized, the Care Plan is live and scheduled!
    ​


💡 Tips & Best Practices

  • ✅ Add more appointment types: Click “Add Scheduling Options” if you’d like to include a second visit type (e.g. Adjustment + Rehab).

  • đŸš« Care Plans cannot be edited or deleted once started in Aloha—any updates must be done from your EHR if available.
