You want to start a Care Plan for a patient
Youâre ready to assign recurring visits for a specific appointment type
You want to ensure patients are on track with their treatment or wellness plan
Care Plans are a powerful tool to help patients stay consistent, organized, and on the path to progressâwhile making your team's day more structured!
â How to Create a Care Plan
Go to the CONTACTS tab and select âShow Contactsâ
This will bring up your full contact list.Search for the patient
Use the search bar in the top-right to find the patient you want to assign the Care Plan to.Click their name and select âCreate Care Planâ
Youâll now see a âCreate Care Planâ button on their contact page.Choose the appointment type
This is the type of visit the patient will be scheduled for throughout their plan.
âSelect the provider
Pick the doctor or practitioner that will see this patient.Click âStart Care Planâ to begin
Now you can begin scheduling their visits.
âSet the appointment times and build out the schedule
Click âStart Care Planâ again to finalize
Once finalized, the Care Plan is live and scheduled!

â Add more appointment types: Click âAdd Scheduling Optionsâ if youâd like to include a second visit type (e.g. Adjustment + Rehab).
đ« Care Plans cannot be edited or deleted once started in Alohaâany updates must be done from your EHR if available.
